HOW TO SCHEDULE AN APPOINTMENT

Please fill out the form. Without us having this information the system will not allow you to schedule.

Once completed you will get an email within 24-48 with two different links. The first link will be the new patient forms. This will need to be completed before for your first visit.

The second link will be for enrollment in the online scheduling feature for Golden Customer Care employees and their families only. Once this is completed you will be able to schedule appointments on-line. When you schedule, YOUR FIRST APPOINTMENT NEEDS TO BE SCHEDULED AS A NEW PATIENT.

Insurances Accepted

Adjustments are $45.00 if not utilizing insurance. If wanting to bill insurance we will need to verify the policy and will let you know if there is an active chiropractic benefit and how much you will owe. HSA/FSA are accepted.
